NOTICE -- Student Visitors

Approval for student visitors must be obtained in advance.  All student visits are approved by administration.

From time to time our former student like to come back and visit our school when their school is not in session.   Please be advised that if a student wishing to visit our school is currently attending a school that is closed for health related reasons, students wishing to visit our school will not be allowed to visit our school during that time period.

Students visiting our school will be expected to follow the classroom and school routines as well as all behavior expectations while they are here.  Administration reserves the right to limit the number of students visiting on any given day.

MUNISING MEMORIAL HOSPITAL INFLUENZA /H1N1 VACCINATION CLINIC

Please note: AuTrain-Onota Public Schools does not make endorsement(s) regarding the  H1N1 vaccine.

AuTrain-Onota Public Schools families are receiving an informational packet regarding the Munising Memorial Hospital Influenza/H1N1 Vaccination Clinic for our students.  The informational packet is being sent home with students this date (November 4, 2009).  Please be advised that this clinic is ONLY for AuTrain-Onota Public School students (Preschool - Eighth grade).  Distribution will begin with the youngest children first.  Munising Memorial Hospital will be scheduling the clinic will be held at our school.

Parents and Guardians are receiving the following information in their packets:  Cover letter, Munising Memorial Hospital Influenza/H1N1 Vaccine Informed Consent form, Client information check box form, including Refusal to Consent to Vaccination and MCIR Seasonal Influenza Scan form, 2009 H1N1 Nasal Influenza Vaccine "What You Need to Know" information sheet, and the 2009 H1N1 Nasal Influenza (the 'flu shot') "What You Need to Know" information sheet.

Munising Memorial Hospital will be scheduling the Clinic, which will be held at our school, based on the time they receive a supply of the vaccine.  Mr. Jim Van Eck, Employee Health Coordinator for Munising Memorial Hospital is our contact person.

Parents and Guardians are asked to real all of the provided information.  It is a parent choice to vaccinate or not vaccinate your child.  If you wish to have your child vaccinated, please fill out and return BOTH forms.  If you wish to 'opt out' of the vaccination, please return the form with the "Refusal To Consent" filled in. 

Please be advised that only students with permission to vaccinate will receive the vaccination.  If no signed form is returned, your child will not be vaccinated.  Consent forms must be returned to the school office by Monday, November 9, 2009. 

If you have general questions pertaining to the clinic, please call the school office.  Medical related questions should be directed to your health care provider.
